As Guardians, SCARC Guardianship Services gets to know each person in order to make sure that our legal responsibility to be a surrogate decision maker is done so collaboratively and with each person’s best interest at heart. We are responsible for decision-making while supporting choice and self-determination, dignity and respect. Below are examples of the case management services we provide, including a minimum of one monthly in-program visit. We ensure quality of life in all areas including health, housing, education, employment, recreation and community living.
- Work closely with residential and day program service providers;
- Determine and monitor residential placement;
- Attend regular and emergent medical appointments;
- Consent to and monitor medical treatment;
- Monitor and advocate for services and programs;
- Monitor and advocate for services related to each person’s safety in the community;
- Advocate and plan for employment in the community;
- Maintain connection to extended family of each individual;
- Make end-of-life decisions;
- Act as representative payee;
- Maximize independence in least restrictive manner, and
- Report to the court about the guardianship status at least annually
